In accordance with federal law, the Office of the County Officers Fulton County is no longer required to automatically mail Form 1095-C to all full-time employees. This form provides information about the health care coverage offered to you during the tax year. You have the right to receive a copy of your Form 1095-C upon request.

To request a copy, use one of the following methods:

Once your request is received, the Office of the County Officers Fulton County will provide your form within 30 days of the request.
